Neuropathy almost always requires a polytherapy approach, meaning several solutions are needed to solve your problems. While I often recommend getting a wide range of labs done to see exactly what may be causing your symptoms, when it comes to neuropathy these are the first four labs I always recommend my patients check out. If you are suffering from neuropathy, reach out to me. Together we can come up with a treatment plan that will eliminate your neuropathy and get you back to playing with your kids, golfing, or whatever other activities your neuropathy is preventing you from enjoying. Lab 1: Mold Many people suffering from a chronic neuropathy condition have a mold infection in their system. These infections can come from contact to mold as many as ten years prior. Mold in your system causes chronic inflammation. Inflammation restricts your nerves from receiving proper nutrition, and prevents them from working properly. This in turn causes neuropathic symptoms. I have had patients with extremely high mold levels. Working with me, the patient was able to get their mold infection eradicated, and saw their symptoms disappear in the process. When doing this lab work, levels should be as close to zero as possible, but anything under 200 is ideal. Lab 2: Fasting Insulin High levels of insulin cause nerves to degenerate. Insulin is essentially like glass shards flowing through your blood. As you can imagine, this causes a lot of inflammation. As described above, inflammation can cause your nerves to degenerate and malfunction. Neuropathy happens because your nerves are not working properly. Since insulin is so closely tied to overall levels of inflammation in your body, it is a key marker to test for in lab work. Look for fasting insulin levels between 5 and 6. Lab 3: C-Reactive Protein C-Reactive protein is a marker that signals levels of inflammation throughout the body. Ideal levels of this protein are less than 1. I have had patients with levels as high as 13 or more. By treating their problems which raised this protein, we were able to lower their markers and neuropathy symptoms. Lab 4: Homocysteine Homocysteine is a marker which will damage your blood vessels and capillaries. This reduces your body’s ability to provide nutrients and maintenance to your nerves, causing them to degenerate. High homocysteine can be a genetic problem, in addition to lifestyle causes. Normal levels of homocysteine in your body should be between 5 and 6. High levels of this marker can signal damage being done to your circulatory system, and should be treated quickly.     Fluoroquinolone Toxicity Syndrome: How Antibiotics Affect Mitochondria Schedule a Discovery Call You may have come across my videos or read on my website about fluoroquinolones causing mitochondrial dysfunction, which leads to a variety of symptoms. In this blog post, I'll break down the process of how and why fluoroquinolones kill both bacteria and mitochondria, as well as the implications for your body. Additionally, I'll explain the role of mitochondria in your body, their location, and why they are so important. Antibiotics and Their Bacterial Targets ​ Fluoroquinolones are a class of antibiotics, and each class kills bacteria in a unique way. For instance, penicillin, which most people are familiar with, destroys the bacterial cell membrane, making it impossible for the bacteria to survive. Over time, bacteria developed resistance to penicillin and other antibiotics. As a result, fluoroquinolones were created to combat these resistant bacteria by attacking their DNA. The fluoride in fluoroquinolone molecules enables this process, as it has an affinity for bacterial DNA. ​ Understanding DNA and Its Role in Cells ​ Every cell in your body, as well as every bacterium and mitochondrion, contains DNA. DNA informs the cell about its identity and function. Without DNA, cells start to malfunction and eventually die. Fluoroquinolones latch onto the DNA and inhibit replication, which spells doom for the cell. To illustrate this, imagine making a thousand copies of an original photo, each with a slight degradation. While you might not notice a difference between consecutive copies, comparing the original to the thousandth copy reveals a significant drop in quality. This is what happens to cells when there's an issue with DNA replication. Fluoroquinolones Can't Tell the Difference Between Bacteria and Mitochondria ​ Research indicates that mitochondria were once bacteria. At some point during evolution, bacteria within cells became beneficial to humans and eventually evolved into the mitochondria found in our cells today. As you can imagine, this means bacteria and mitochondria still share many similarities. They are the same size, reproduce in the same manner, and both have their own DNA arranged loosely within the cell membrane. ​ The Abundance of Mitochondria and Their Vulnerability Thousands of mitochondria are present in each cell within the human body. Some organs, like the heart, contain even more of them, as they are always active and require more energy to function. However, mitochondrial DNA is more exposed compared to the DNA inside a cell's nucleus, which is tightly packed around histones and organized into chromosomes. In contrast, mitochondrial DNA floats freely within the mitochondria, making it less secure and more vulnerable. And, as mentioned earlier, mitochondria bear a striking resemblance to bacteria. The Function of Mitochondria and Their Vulnerability Unfortunately, fluoroquinolones, which are designed to kill harmful bacteria and treat illnesses, can't differentiate between harmful bacteria and your mitochondria. They attack mitochondrial DNA, inhibiting replication. Without DNA, the mitochondria can't perform their essential function: producing energy. ATP synthesis, or energy production, is the primary role of mitochondria. If your mitochondria are affected, it will impact energy production, which in turn affects the cell, then the organ, and ultimately, you. Factors Damaging Mitochondria and Their Impact There are several ways in which mitochondria can be damaged. Chronic inflammation, aging, or being "Floxed" are examples of factors that can harm your mitochondria. Additionally, damaged mitochondria can affect various cells throughout the body. Nerve cells, for instance, rely on mitochondria to function. So do your small intestine and mucus membranes. Every cell in your body contains mitochondria – not just one or two, but hundreds to thousands. Therefore, when you experience symptoms like leaky gut, dry mouth, or neuroinflammation, it could be due to mitochondrial damage. Fluoroquinolones Affect You on a Cellular Level The purpose of this blog post is to emphasize that when you take fluoroquinolones, they profoundly impact your body at the cellular level. The mitochondria, which produce energy for your cells, and their DNA are being damaged or destroyed. These medications are highly absorbable and can easily penetrate the membranes of our cells and the membranes of our mitochondria.     Your Functional Medicine Guide To Underlying Thyroid Dysfunctions + How To Heal Schedule a Discovery Call The thyroid is something that is often the source of many health problems. Low thyroid function, or hypothyroidism , affects around 20 million Americans, and 1 in 8 women will experience a thyroid issue in her lifetime. ​ The most common cause of low thyroid function is the autoimmune condition, Hashimoto’s disease. If you are faced with this condition there are a few underlying factors to consider when looking to treat your symptoms. ​ Hashimoto’s disease or autoimmune thyroiditis and was the first autoimmune disease to be discovered in 1912. Your immune system works to keep you healthy by fighting off viruses and bacteria that threaten your body. However, autoimmune conditions occur when your immune system mistakes parts of your body as a threat and ends up attacking itself. This is considered an autoimmune response. ​ To diagnose Hashimoto’s, you’ll run thyroglobulin antibodies (TGB Ab) and thyroid peroxidase antibodies (TPO Ab) tests. Normally, doctors don’t run these tests because thyroid hormone replacement drugs would be the treatment regardless if you have true primary hypothyroidism or Hashimoto’s. Since the autoimmune response has both active and dormant states, it is vital to look at your entire health history in the case of a false negative test. ​ Out of all the hypothyroid cases in America, 90 percent are autoimmune . (1) In fact, thyroid related autoimmune conditions affect an estimated 8 percent of the population. In conventional medicine it doesn’t matter if you have a dysfunction with thyroid hormone conversion, primary hypothyroidism, or an autoimmune disease, they will look at treatment the same. However, when it comes to Hashimoto’s the thyroid isn’t actually the problem, making thyroid hormone replacement medication unhelpful. The thyroid is merely the victim of the immune system’s attacks. ​ Unfortunately, hypothyroidism is so common that it can often be disregarded. “Here’s your prescription. And there’s the door.” The reality, though, is that if your thyroid isn’t working well, nothing in your body will. This master gland that is located in your neck controls many, far-reaching facets of your health, and every cell in your body contains a thyroid hormone receptor. 13 Signs of Thyroid Dysfunction - Thyroid Health 1. Your bones are weak If you have weak bones, have your thyroid checked. Abnormal thyroid function can decrease (2) bone mineral density. Serum calcium will typically be in the “normal” reference range with hypothyroidism, but outside of the functional, optional range. Over time, this decreasing bone density can lead to fractures. ​ 2. You have gut problems Among many of the other processes controlled by thyroid hormones is digestion, and low thyroid function can reduce the movement (3) of your intestines, which is of course an essential part of healthy digestion. Poor thyroid function also compromises your body’s ability to absorb nutrients. Healthy thyroid function dampens gut inflammation, so low thyroid function can furthermore be linked with gastric ulcers and leaky gut syndrome. ​ 3. You are experiencing a low sex drive Although many people think a reduced sex drive is just due to aging, in fact it may be a symptom of abnormal hormone function. Thyroid hormones affect the metabolism of estrogen and testosterone in the body and thyroid hormone problems can cause low libido in both men and women. Also, hypothyroidism in men is linked (4) with erectile dysfunction. ​ 4. You are having trouble with your weight When your thyroid hormones are low, your body will be less able to break down fat, which can quickly add pounds or make you resistant to weight loss despite your efforts at dieting or working out. These efforts don’t address the underlying problem because weight gain isn’t the cause of your problems, but a symptom of something else that is not being addressed. Weight loss happens when you get healthy to lose weight, not the other way around. When you restore thyroid function, weight loss will be the natural byproduct. ​ 5. Your energy is lacking In addition to slowing your ability to burn fat, hypothyroidism will also decrease your energy, causing debilitating fatigue. ​ 6. You develop blood sugar problems When your body is in a low thyroid state, it decreases your body’s ability to absorb glucose or blood sugar. Glucose needs to get into your cells in order to create ATP, your cellular energy source. Despite sluggish glucose metabolism, many people struggling with low thyroid hormones can feel hypoglycemic , (5) like they have low blood sugar. Because the cells are not getting the glucose they need, you can feel like you are hypoglycemic even with normal looking blood sugar labs. This vicious cycle of hormonal dysfunction can lead to metabolic syndrome or insulin resistance. ​ 7. You tested positive for high cholesterol and triglycerides Sluggish thyroid hormone levels decrease the breakdown (6) of cholesterol, which can lead to elevated total cholesterol and triglycerides. While high cholesterol alone is a poor predictor for heart attack and stroke risk, elevated triglycerides is an accurate marker for these life-threatening health emergencies. ​ 8. You suspect you have adrenal fatigue When you have hypothyroidism, it puts stress on your hypothalamic-pituitary-adrenal (HPA) axis. Because of the lack of glucose and energy getting to the cells, the brain signals the adrenals to produce more cortisol in an attempt to get more energy into the cells. This further complicates your hormonal health picture and can lead to HPA axis dysfunction and eventually, adrenal fatigue. ​ 9. You are overloaded with toxins Your liver and your gallbladder do the heavy lifting when it come to the removal of toxins you ingest or that are the by-products of metabolism. Poor thyroid health can inhibit optimal function of your liver and gallbladder, which can significantly decrease your body’s ability to rid itself of toxins. This may also cause nonalcoholic fatty liver disease . (7) Impaired detoxification pathways are common in patients dealing with thyroid problems. ​ 10. You have estrogen dominance or deficiency Estrogen comes in the form of three metabolites: Estrone (E1), estradiol (E2), and estriol (E3). You need just the right amount of each of these for optimal health, but when your thyroid isn’t working well, it can unbalance your estrogen metabolite ratio. ​ 11. You experience brain problems Poor neurotransmitter expression and an increased risk of mental health problems such as depression, anxiety, and bipolar disorder can be linked to hypothyroidism, due to the fact (8) that the brain contains a predominance of thyroid receptor sites. Also, people with depression had higher rates of thyroid conversion impairments, or low T3 syndrome, according to one 2014 study . (9) ​ 12. You get hot flashes or cold spells When your thyroid hormone levels are low, it affects your body’s temperature control. This can cause you to feel cold all the time, or have night sweats and hot flashes. ​ 13. You are losing hair Since thyroid hormones are one determining factor of metabolic rate and nutrient absorption, thyroid dysfunction can lead to hair loss . (10) Regaining hair health means optimizing thyroid hormone levels. ​ ​ ​ ​ ​ ​ ​ ​ ​ ​ ​ ​ ​ ​ Underlying Causes For Thyroid Dysfunction In order to successfully treat hypothyroidism, we need to understand what triggers the immune system to attack the thyroid in the first place. Only then can we begin to mitigate these triggers to start a path to true healing. ​ 1. Stress Research has confirmed the stress-thyroid connection. Two studies, one from 1991 (11) and the other from 2001 , (12) both found that autoimmune thyroid patients had more stressful life events before their diagnosis, compared to control groups. This could be due to the effect stress has on your thyroid hormone function. Your main stress hormone, cortisol, can block conversion of T4 to the active T3, and can also increase the unusable reverse T3 (rT3), further confusing the delicate balance of thyroid hormones that are so essential for good health. Many of my patients discover that their thyroid problems started after a stressful time in their lives. ​ Be consistent with stress-busting practices like mindfulness meditation and yoga. These activate your body’s parasympathetic nervous system, which will make you feel more zen, while also boosting healthy thyroid function. ​ 2. Food I ntolerances If you are predisposed to an autoimmune condition, certain foods can cause your immune system to overreact. Specific foods such as grains, gluten, soy, and dairy can contribute (13) to inflammation which can further perpetuate symptoms and autoimmune flare-ups. ​ You don’t have to have celiac disease to have a gluten intolerance – the inflammatory effects of gluten were shown (14) to last up to six months each time you eat it, even in those without celiac disease. ​ Imagine what happens when you eat it every day! You can see the far-reaching effects of this protein and its resultant long-term inflammatory response. Conventional medicine practitioners typically don’t run the proper tests to see if this is a trigger in individual cases, but in functional medicine, we often see this connection. ​ I run a full food intolerance panel to help you determine which foods are an issue for you. ​ 3. Nutrient Deficiencies Your body relies on a certain amount of specific nutrients for your immune system to function optimally. Deficiencies in selenium (15) and vitamin D (16) have been linked to poor immune function in people with autoimmune thyroid conditions. ​ 4. Gut Problems Close to 80 percent of your immune system is found in your microbiome. A compromised gut like in leaky gut syndrome or bacterial imbalances will contribute (17) to poor immune health. Also, 20 percent of your T4 is converted to T3 in the gut, and an imbalanced, unhealthy microbiome can inhibit this process. Gut labs can determine your level of gut dysfunction to see if this is a factor in your health case. ​ 5. Toxin Exposure Our world has become increasingly toxic. Studies have shown that chemicals and heavy metals can cause an autoimmune response (18) against the thyroid. You can measure your toxicity levels through labs such as a urine heavy metal panel. ​ 6. Estrogen Imbalances Proper levels of hormones are important for optimal health. An imbalance of estrogen in women has been shown to trigger autoimmune thyroid problems. ​ 7. Blood Sugar Dysregulation PCOS, insulin resistance, or other blood sugar problems can contribute and perpetuate Hashimoto’s. ​ 8. Iodine Levels While iodine is necessary to make thyroid hormones, excess amounts can actually exacerbate Hashimoto’s symptoms . (19) Research has also found (20) an increase in thyroid antibodies with iodine supplementation. Since iodine is an active ingredient in many thyroid supplements, it just goes to show that what works for one person, doesn’t always work for the next. ​ 9. Infections Infections, including bacterial, parasitic, fungal, and yeast, are associated (21) with just about every autoimmune condition. This could be due to the fact that the majority of our immune system is produced from the gut microbiome, so when infections enter the gut, they can alter immune activity. ​ This can become a trigger for autoimmune thyroid disorders (AITD). In the above study, 36% of Graves’ disease disease patients had evidence of a recent viral or bacterial infection. Avoiding or immediately treating infections may prevent this chain of events. ​ 10. Artificial Sweeteners Artificial sweeteners like saccharine (Sweet’N Low), sucralose (Splenda), and aspartame (Equal and NutraSweet) are in many diet and zero-calorie drinks and foods, but even though people may believe that reducing calories with these products is good for health, the fact is that simply eliminating artificial sweeteners and diet soda alone may reverse Hashimoto’s. The mechanism seems to be their proven negative effect on the microbiome. ​ 11. Smoking By now, everyone knows that smoking is destructive to good health, for genetically susceptible people, it can be especially disastrous. Multiple studies have examined the effects of smoking on people with autoimmune thyroid disorders. One study (22) observed an increase in autoimmune hypothyroidism in women smokers, while another study showed an increase in Graves’ disease in smokers. ​ But studies have shown that a decrease (23) in thyroid function could be reversed by taking n-acetylcysteine (NAC), and by increasing the super-antioxidant glutathione. ​ 12. Low Vitamin A Low vitamin A can spell trouble for your thyroid because this fat-soluble vitamin has been shown (24) to boost T3 levels and normalize TSH. ​ True vitamin A, called retinol, is only found in animal products like fish, shellfish, fermented cod liver oil, liver, and butterfat from grass-fed cows . Plant carotenes, a precursor to vitamin A, are found in sweet potatoes and carrots, but the conversion rate to the usable retinol is very weak. In fact, research suggests (25) that just 3 percent of beta-carotene gets converted in a healthy adult. So eat up those animal fats! ​ 13. Low Selenium Selenium is essential (26) to convert T4 to T3 in your liver. Selenium also protects (27) against autoimmune thyroid problems. Eat a variety of nuts and seeds, especially Brazil nuts, as well as oysters, another good source of selenium. ​ 14. Low Iron Iron is needed (28) for the production of thyroid peroxidase, the enzyme used to make your thyroid hormones. If you have low iron, the most important thing to discover first is the underlying problem that’s causing the iron deficiency. Healing your gut is essential for healthy nutrient absorption, especially iron. Once the gut is healed, iron-rich foods like grass-fed beef , liver, and spinach can be effective – as can cooking with cast-iron cookware. ​ 15. Low Copper Healthy copper levels were found (29) to increase total T4 and T3 levels. The best way to get bioavailable copper is by eating grass-fed liver and oysters. Sesame seeds are a good plant source as well. ​ 16. Hormone Imbalances Your hormones are all connected, and the ripple effect from dysfunction in one endocrine gland outputting any hormone can negatively affect your thyroid. Low estrogen , (30) insulin resistance , and low testosterone (31) were all found to inhibit thyroid function. Depending on your individual hormone problems, solutions will vary. ​ 17. Pain Being in chronic pain has been shown (31) to suppress deiodinase, the liver enzyme that converts 80 percent of your T4 into your active T3. ​ There is no simple answer for this. No one chooses to be in chronic pain, but there are many therapies that may help. I have seen some success with liposomal turmeric, which has great bioavailability compared to the standard version of this anti-inflammatory natural product. ​ The 6 Different Types of Underlying Thyroid Problems While some are more common than others, there are actually a few different thyroid problems you could be experiencing due to the fact that thyroid physiology is complex and unique to the individual. So when you have thyroid-related symptoms such as fatigue or weight gain, your problem may be more complex than something that can be fixed with a simple thyroid hormone replacement drug. (And no, thyroid dysfunction is not a sign of “just getting older,” as you may have been told.) You may even have thyroid symptoms despite “normal” labs. Unfortunately, there’s no such thing as a magic thyroid-fixing pill for everyone. Instead of diagnosing “low thyroid,” functional medicine doctors prefer to make a more comprehensive diagnosis, to gain insight into why someone is struggling with the symptoms they have. ​ Here are six hidden thyroid problems we need to start talking about and what to do about each one: ​ 1. Autoimmune T hyroid Problems Research estimates (32) that up to 90 percent of all cases of hypothyroidism are autoimmune in nature, with the most common manifestation being Hashimoto’s disease. When this happens, your thyroid is not the source of the problem, but the victim of a misguided immune system attack, in which your body mistakes your thyroid for a virus. ​ In mainstream medicine, you typically receive the same thyroid medication whether you have true primary hypothyroidism or an autoimmune disease attacking your thyroid, but in functional medicine we tend to treat autoimmune thyroid problems more naturally and can be different for each individual – since every person has their own unique biochemistry. ​ Labs to run: Thyroid peroxidase and TBG antibodies What to do: There’s a lot you can do to naturally manage this condition. I recommend healing the gut with tools such as the elimination diet as well as taking in natural compounds such as Baicalin (33) to promote regulatory T cell (Treg), to balance the immune system. ​ 2. Thyroid Resistance This thyroid dysfunction is also not a primary thyroid problem but a cell receptor issue. Every cell of your body depends on thyroid hormones to function the way they should, but if your cells receptor sites are blunted because of inflammation or toxins, your body will feel the effects of hypothyroidism despite having adequate hormone production, since those hormones aren’t getting where they need to go. In this case, your TSH test will be “normal” but you’ll feel miserable. ​ Labs to run: Markers such as CRP and homocysteine as well as reverse T3 can shed light on inflammation levels as well as hormone receptor site availability. High triglycerides and low HDL can be signs of insulin resistance, which is another clue because it typically occurs with thyroid resistance. What to do: Liposomal turmeric and resveratrol are two of my favorite supplements for quelling excessive inflammation. ​ 3. Elevated T hyroid-Binding Globulin This is a common scenario when a woman has high estrogen levels. I also see it in women using birth control pills or estrogen creams. All hormones get transported throughout the body on protein carriers. The thyroid’s specific transport carriers are called thyroid-binding globulins (TBG.) When TBG levels are elevated, this could cause an increase of protein-bound thyroid hormones, which can’t be used by your body. ​ Labs to run: TBG, Free T3, and T4 as well as an expanded female hormone panel. What to do: Clear excess hormones by supporting detoxification pathways with herbs such as milk thistle, and by supporting methylation pathways with activated B vitamins. ​ 4. Hypothyroidism Second to Pituitary Hypofunction Your thyroid only does what your brain tells it to do, and if your pituitary gland isn’t working well (this could be due to chronic stress, chronic infections, or even a benign pituitary tumor), the hypothalamic-pituitary-thyroid (HPT) axis will malfunction. This is the communication line between your brain and thyroid. When the pituitary gland, which sits at the base of your brain, isn’t communicating with your thyroid gland, then the thyroid may be just fine. It’s just that nobody is giving the signal that it’s time to get to work! ​ Labs to run: Look for low levels of T4 and T3 despite a normal-to-low TSH, and possibly viral labs, such as the one that checks for the Epstein-Barr virus (EBV.) What to do: To support pituitary health and optimal HPA communication, I recommend natural brain-immune support tools, such as astragalus, olive leaf, rubidium, sage leaf, L-lysine, zinc, and vitamin C. ​ 5. Thyroid U nder-Conversion This pattern, also called low T3 syndrome, is common with chronic adrenal stress and its related elevated cortisol. The majority of thyroid hormone comes in the form of T4, which is largely metabolically inactive and has to be converted to T3 for use in the body. It’s estimated that 80 percent of thyroid conversion happens in your liver and 20 percent in your gut. ​ Through this process, a special enzyme called 5′ deiodinase takes one iodine molecule off of T4 to make it the active T3. This pattern has been covered in medical literature for years but is not effectively alleviated by the thyroid replacement hormones. Most thyroid drugs are synthetic T4, which isn’t the problem in this pattern. ​ Labs to run: Free and total T3, liver enzymes, microbiome testing, and nutrient testing of selenium, magnesium, and zinc. What to do: In addition to getting your adrenal fatigue, gut, and liver health under control, foods rich in selenium, zinc, and magnesium help make the enzyme needed for healthy thyroid conversion. Brazil nuts and shellfish, such as oysters, are some of my favorite options. ​ 6. Thyroid O ver-Conversion This thyroid pattern is sometimes found in people with elevated testosterone levels, insulin resistance, type 2 diabetes, and in women with PCOS (polycystic ovarian syndrome.) You would think that if the body produces more T3, the metabolically active form of thyroid hormone, it would be a good thing. However, as with just about everything, too much of a good thing isn’t such a good thing. Over-conversion of T3 can overwhelm your cell receptors, leading them to become resistant to the T3, so even though you have a lot, once again, you aren’t able to use it. ​ Labs to run: Free and total T4 and T3, free and serum testosterone, fasting blood sugar, and HbgA1c. What to do: In addition to eating a clean diet consisting of lots of vegetables, fruits, meats, and healthy fats, I recommend natural medicines such as alpha-lipoic acid and chromium supplements, along with cinnamon, all of which can act as blood sugar balancers. As for general thyroid health, sea vegetables such as dulse, nori, and arame are great nutrient sources, including the vitally important iodine. You need iodine from foods to fuel thyroid hormone production. ​ A Functional Medicine Guide To Thyroid Labs Diagnosing a thyroid issue isn't easy because lab test results can be confusing and misleading. If your doctor only tests for your level of thyroid-stimulating hormone, or TSH, then you will only get treated (with a synthetic thyroid hormone like Synthroid or levothyroxine) if your labs are out of range. If your labs come back "normal," you will probably be sent home with a shrug or an assurance that you are fine. In my view, this is a serious problem. ​ How Labs Determine “Normal” Ranges If you have the symptoms of low thyroid hormone but your TSH is “normal,” something is not being addressed. Part of the problem starts with the reference range for what is considered normal. How do we get the “normal” lab range? The reference range is based on a statistical average of the population getting tests out of the lab. This is why (other than for vitamin D and cholesterol levels) lab reference ranges will vary depending on the lab. ​ The people who get tested in labs – in other words, the population of that lab – are obviously statistically not the healthiest segment of the population. Otherwise, they wouldn’t be getting lab tests. That means if the lab results are “normal” despite your symptoms, what your doctor is essentially saying is you’re just like a lot of other sick people! ​ In functional medicine, we look at a much narrower range of normal, taking this into account. We want your labs to look like a bunch of healthy people whose bodies function optimally and who feel great. ​ Thyroid Labs: The Functional Medicine Version A functional medicine look at your thyroid is more in depth than just a TSH and maybe a T4. We do a broader range of tests and look at a narrower range of optimal results. Here are some of the labs I run on my patients, what they actually mean, and what kind of results are ideal: ​ TSH Thyroid-stimulating hormone is released from your pituitary gland to communicate with your thyroid. If your TSH is high, it’s sort of like your brain shouting at your thyroid to work harder. Research has linked (34) a lab “normal” TSH of 2.5-3.5 mlU/mL with a 69% risk of dying from a heart attack or stroke. Now you can see why the optimal (functional) range is so important for your health. Forget the larger reference ranges and aim for what’s optimal: Lab range: .45-5.5 mlU/L Optimal range: 1.8-2.5 mlU/L ​ Total T4 T4 is mostly metabolically inactive in the body and has to be converted to T3 to be usable. This lab gives you a total of unbound and bound forms of T4. Hormones have to be unbound from the protein carrier to be used by your body. Because of this, this lab doesn’t give us the activity of T4 when measured alone. T4 is measured in conjunction with a T3 uptake. Lab range: 4.5-12 mcg/DL Optimal range: 6.0-12.0 mcg/DL ​ Total T3 This lab shows us the total amount of the metabolically active thyroid hormone. It allows a doctor to check your body’s ability to convert T4 to T3 and to rule out an overactive thyroid. Lab range: 80-200 ng/DL Optimal range: 100-180 ng/DL ​ Free T4 This will tell you the levels of free or active form of T4. This will be low in cases of hypothyroidism but can be normal in subclinical, early stages of thyroid dysfunction. Lab range: 0.8-1.8 ng/DL​ Optimal range: 1.0-1.5 ng/DL ​ Free T3 This is the more active, usable form of your thyroid hormone. Low T3 syndromes are a common dysfunction that I see in practice, and a low level of this hormone is strongly linked (35) to a higher risk of heart attack. The problem, if you’re taking a synthetic T4 medication, is that your body isn’t converting the hormone properly into T3, though there are many reasons your thyroid medication may not be working. Lab range: 2.3-4.2 pg/mL Optimal range: 3.0-4.0 pg/mL ​ Reverse T3 Chronic stress and high cortisol can raise levels of reverse T3, which is an unusable form of the thyroid hormone. Reference range: 8-25 ng/DL Optimal range: 9.2-20 ng/DL ​ Thyroid Antibodies High levels of thyroid antibodies show an autoimmune attack against the thyroid. The overwhelming majority of low thyroid cases are on the autoimmune spectrum, the most common being Hashimoto’s disease. ​ Thyroid Peroxidase (TPO) Ab optimal range: 0-15 IU/mL Thyroglobulin Ab optimal range: 0-0.9 IU/m L ​ In addition to these thyroid-focused labs, functional medicine practitioners are likely to recommend additional labs to address your microbiome, immune system, and other hormones. These should also be considered for the most complete picture of what is going on with your health. ​ Getting the big picture from lab work is a solid first step towards finding out exactly what is going on with your thyroid. What conventional doctors may not tell you is that there are many thyroid dysfunctions that typically don’t show up on labs, and that throwing synthetic thyroid medication at the problem will, in many cases, not do enough to relieve symptoms. You can do more to help. ​ The Uncomfortable Truth Behind Thyroid Medications Treatment of thyroid disorders in conventional medicine is, in my opinion, inadequate. Patients with hypothyroidism are typically given a thyroid replacement hormone as medication and care stops there. Because of this, some people still experience low thyroid symptoms despite medication and “normal” labs. The reason is that many thyroid problems are not actually problems of inadequate thyroid hormone. My patients often ask me which thyroid medication is right for them or whether they even need to be on one in the first place. Here is my functional medicine perspective. ​ What are my options for thyroid medicines? Thyroid replacement hormones are one of the most commonly prescribed medications. Here is a rundown of the most common prescriptions: ​ Synthetics Levothyroxine: This is the most common thyroid hormone. This synthetic T4 hormone comes in over 30 different brand names such as Synthroid and Levothroid. ​ Liothyronine: This is the generic name for synthetic T3, the active form of the thyroid hormone. The most common brand name is Cytomel. ​ Natural Sources Armour Nature-throid Westhroid Compounded thyroid hormones ​ These naturally sourced supplements come from pig thyroid tissue. Because they are natural, they contain both T4 (the less active hormone that has to be converted in the liver and gut) and T3 (the more biologically active form) hormone. ​ Many primary care physicians and endocrinologists are only comfortable prescribing the synthetic replacement hormones because decades ago, there was less standardization with natural options. Today, this is not as much of a concern and when thyroid hormone replacement is necessary, I prefer natural options (these are obviously not vegan friendly, if that is a concern for you). ​ However, these drugs aren’t right for everyone, synthetic or natural, since everyone is different and there is no “one size fits all” or magic pill for thyroid dysfunction. This brings me to the next question I am frequently asked: ​ I’m on a thyroid medication but it isn’t working for me. Why not? If you’re still having low thyroid symptoms despite medication, it’s probably not that “you’re just depressed” or “just getting older.” Here are some other factors you should consider: ​ 1. Inactive Ingredients Thyroid drugs are pretty comparable as far as their active ingredients, with the biggest variable being the inactive ingredients. Because everyone is different, and many thyroid problems are autoimmune in nature, the fillers used in these pills may be causing a negative response. Drug formulas can change, so it’s important to check your individual medication. If you think you might be having a reaction, immunological labs can uncover certain intolerances you may be having. ​ Some common inactive ingredients found in thyroid medications that you may react to include: dyes cellulose lactose other sugars extra iodine (excess iodine can trigger autoimmune thyroid problems) corn starch modified food starch (may contain gluten, which can trigger autoimmune thyroid problems) ​ 2. “Normal” lab ranges may not be normal On your lab, you’ll see a “Reference Range.” This range is what’s considered normal in mainstream medicine. That range is based on a bell curve – a statistical average of the population of that particular lab you went to. People who go to labs are typically sick people, so if your result comes back within that reference range, it doesn’t automatically mean it’s at the optimal level for you. ​ In functional medicine, we look at a narrower and more personalized range of “normal,” so you may find that while a conventional doctor says you are “normal,” a functional medicine doctor will disagree. In addition to tweaking this reference range, your functional medicine doctor may also want to run more expansive thyroid tests to help uncover your exact issues. ​ Sometimes there are other thyroid problems that won’t be solved by a pill alone. Thyroid conversion problems and Hashimoto’s disease (autoimmune thyroiditis) are two often overlooked underlying thyroid issues that should be ruled out before defaulting to a pill as the only treatment. ​ Inflammation can significantly inhibit thyroid hormone function. Addressing this means dealing with the underlying causes of inflammation, including a poor diet, stress, and toxins. Thyroid hormone resistance, similar to insulin resistance, is one reason why your thyroid medication may not be working for you. I recommend working with a qualified functional medicine practitioner to uncover and address chronic inflammation. ​ 3. You might be experiencing decreased absorption There are several medications that alter the absorption of activity of T4, a hormone produced by the thyroid. These might include commonly prescribed drugs like antacids, antibiotics, and antifungals, antiarrhythmia medications, cholesterol-lowering medications, diabetic medications, diuretics, hormone replacements, and pain medications. Some foods may decrease the absorption of thyroid hormones, such as soy-based and high-fiber foods. Iron and calcium supplements can also inhibit thyroid hormone function. According to the drug manufacturers, it’s best to take your medications four hours before or after you ingest foods, drugs, or supplements that can affect its effectiveness. ​ 4. You might have other health problems There are other underlying health problems that can both mimic low-thyroid symptoms and also complicate healthy thyroid function. Candida overgrowth, leaky gut syndrome, adrenal fatigue, and other underlying metabolic problems should all be investigated along with thyroid dysfunction, as they are common co-issues. ​ The Real Reasons Your Thyroid Medications Aren't Working First of all, thyroid medication normally comes in the form of a synthetic T4 thyroid hormone. T4 is the main thyroid hormone that your body produces naturally, but it needs to be converted into T3 for it to be used actively in the body. I have found that the overwhelming majority of people struggling with low thyroid symptoms are having some sort of autoimmune response, in which the immune system mistakes the thyroid as a foreign body and attacks it, destroying it so it cannot produce the thyroid hormones your body needs. ​ This immune dysregulation can cause systemic inflammation in the body. Inflammation is a normal response to an injury, but when the source of the inflammation, in this case an autoimmune response, isn’t addressed, the vicious cycle of systemic inflammation is never broken. Adding a synthetic hormone to replace what your thyroid isn’t making is only a drop in the bucket in terms of addressing the symptoms that result from this cascade of dysfunction. Not only that, but your body may not be capable of using the hormone you are taking every day. ​ The only way to actually repair this problem is to address the underlying issues of an autoimmune pattern and inflammation. This is how to restore health. With that in mind, let’s go over five reasons why your thyroid medication may not be working, based on the actual thyroid problem you might have: ​ 1. Decreased thyroid receptor sensitivity Every cell of your body is lined with what is called a bilipid membrane. This membrane is made of saturated fat and cholesterol and your health depends on this membrane’s integrity and functionality. Receptor sites on the membrane act as communicators with hormones, but the presence of cellular inflammation can cause a rigidity of the cell membrane and a “dulling,” or resistance, of the receptor sites . (36) Your body may be producing enough thyroid hormone or you may be on the right dose of thyroid hormone drug, but if your cells aren’t communicating with the hormone, your symptoms will persist because your body cannot communicate with those hormones. ​ 2. Decreased thyroid hormone conversion Inflammation can cause other problems as well. It can decrease the body’s ability to convert the inactive thyroid hormone, T4, to the biologically active and usable form, T3 . (37) This hormone conversion happens primarily in the liver and your gut and is essential for your body to utilize thyroid hormone. When that conversion fails, your symptoms will persist, even in the presence of thyroid medication, which (as I mentioned above) is typically in the form of T4 and needs to be converted to T3, just like the hormones your own body makes. ​ 3. High reverse T3 levels When your body is under chronic stress, it can produce higher levels of reverse T3 , (38) which is an inactive form of the thyroid hormone. Reverse T3 can’t be converted into T3, so it basically takes up space and interferes with the healthy production and conversion of regular T3 – another situation where the problem isn’t a deficiency in thyroid hormone, but a conversion problem caused by chronic stress and inflammation. ​ 4. Decreased brain-thyroid communication Your thyroid doesn’t act on its own. It receives directions from your brain, specifically the hypothalamus and pituitary glands. Think of your thyroid as an employee of the brain, only doing what the brain tells it to do. This interconnected web of communication is called the hypothalamic-pituitary-thyroid (HPT) axis . (39) Chronic stress, autoimmune responses, and inflammation in any of these areas can negatively impact these communication lines, so thyroid hormone production could be impacted by a problem in the hypothalamus or pituitary glands, when the thyroid gland is fine. ​ 5. Selenium Deficiency Although relatively rare in adults, a selenium deficiency can sometimes result in people with chronic inflammation, such as may occur with gastrointestinal disease . (40) Selenium is an essential micronutrient that plays an important role in thyroid hormone conversion and overall function. Dietary sources include nuts, meats, fish, and eggs. ​ What’s the Take-Away? The short answer to the question of why your thyroid medication isn’t working lies in the one common component in all the various ways thyroid dysfunction can happen: Inflammation. Inflammation is the common link between all chronic diseases, so assessing and addressing your own levels of chronic inflammation is an essential first step to restoring thyroid health. A comprehensive health history and lab tests can determine both your levels and underlying sources of inflammation, such as chronic infection, toxicity, food intolerances, autoimmune response, or a combination of these. This is a beginning because without reducing inflammation, optimal health is not possible. ​ Nutrients For Healthy Thyroid Hormones Here is my guide to the essential aspects of thyroid function and how to support each through nutrition. ​ TSH Thyroid-stimulating hormone, or TSH, is released from your pituitary gland to communicate with your thyroid. For healthy, happy TSH, you need four things:​ Protein: Great bioavailable sources of protein are wild-caught fish, grass-fed beef , and blue and green algae. Vitamin B12: A great source of B12 is grass-fed beef. Additionally, B12 can be found (41) in egg yolk, salmon, and organ meats such as liver. Magnesium : Dark, leafy greens like spinach and kale are good sources, as well as nuts and seeds (preferably soaked in water for a few hours then dried before eating). Zinc : Grass-fed beef is the highest food source. Shellfish like lobster, mussels, and oysters also contain zinc. Nuts and seeds are good plant-based options. ​ T4 T4 is another thyroid hormone that is typically inactive in the body and has to be converted to T3 to be usable, except in your brain, which needs straight-up T4 to work optimally . (42) For T4 production you need three essential nutrients: Iodine : The “4” of T4 represents four iodine molecules. The best iodine-rich foods are sea vegetables like dulse, kombu, nori, arame, and kelp. Vitamin B2 : The best sources for B2 are grass-fed liver and lamb. Mushrooms and greens are decent plant sources. Vitamin C : Your thyroid needs to draw iodine in to make T4 through what’s called a symporter, which requires (43) both vitamin B2 and vitamin C. The symporter won’t work (44) unless you have these two nutrients! Oranges and red peppers are the best foods to get your vitamin C. ​ T3 This is the more active, usable form of thyroid hormone, which is converted from T4. Think of T3 as your body’s gasoline. Selenium : Without selenium we won’t convert T4 to T3, and we make more of an unusable thyroid hormone that can actually block (45) thyroid function. The best way to get your selenium is Brazil nuts. I suggest getting your selenium levels tested and eating two to four Brazil nuts (soaked in water for a few hours before eating) each day to get all the selenium you need. ​ Thyroid Receptor Site In addition to the necessary thyroid hormones, you need healthy receptor sites for your hormones to work. Like a lock and key, hormones and their receptor site need to line up and function properly. To keep your thyroid receptor sites in tip-top shape, you need: Vitamin A: Low levels of vitamin A and vitamin D can stop T3 from activating. Grass-fed liver and tuna are the highest sources of true vitamin A, which is only found in animal meat. Sweet potatoes and carrots are good sources for the plant-based vitamin A precursor known as beta-carotene. Vitamin D : Responsible for over (46) 200 genetic pathways, this sunshine vitamin is super important, especially when it comes to supporting thyroid hormone receptor sites. The best food sources of vitamin D are fatty fish like salmon, beef liver, and egg yolks. Even more importantly: Get out in the sun whenever you can!     Autoimmune Diseases Affect 50 Million Americans In comparison, cancer affects up to 9M Americans and heart disease up to 22M Americans. Today, there are close to 100 recognized autoimmune diseases (AD), and an additional 40 disease processes that have an autoimmune component. The NIH estimates annual direct health care costs for AD to be in the range of $100B. I recently learned through social media that people can get angry when you tell them autoimmunity is incurable, but this is because of confusion about the difference between cure and remission. ​ Cure means you have remedied the condition and need no further management or treatment. So far, this has not been possible with autoimmunity as identified in published scientific research. ​ Remission means you have dampened the expression of the autoimmune disease and it is not producing symptoms or damaging tissue. However, relapses can happen. ​ People can frequently drive their autoimmunity into remission with dietary, lifestyle, and nutritional factors to improve the quality of their lives. However, it’s important to know that putting your autoimmunity into remission — and keeping it there for long periods — is not a cure. I have seen patients become very demoralized when they relapsed because they believed they had cured their autoimmunity. ​ Even many practitioners don’t understand this and unknowingly give patients false promises. Understanding how autoimmunity works and having healthy expectations is essential to managing your condition. When you understand that relapses are a possibility, you are better equipped to deal with them when they happen and take the necessary steps to get back into remission more quickly. ​ Understanding autoimmune relapse and remission Autoimmunity is a medical condition that occurs when a person’s immune system attacks and destroys their own body tissue. Any tissue in the body is subject to an autoimmune reaction. We identify autoimmunity through tissue antibodies in a blood test. Antibodies are immune cells that tag tissue to alert the immune system to attack it. ​ Relapse happens when the symptoms of an autoimmune disease flare up. This does not point to failure on the part of the patient or healthcare professional—many things can trigger a relapse. A severe stressor, an accidental exposure to an inflammatory trigger, emotional duress, or hormonal shifts are examples of things that can trigger relapses. Triggers sometimes change as we age and our immune systems shift. ​ ​ Navigating autoimmune relapses Despite the dedicated efforts of top scientists and researchers, autoimmunity is still considered incurable. Instead, the goal of every clinician, healthcare professional, and patient should be to put autoimmunity into remission and maintain remission as long as possible. ​ However, understand that relapses happen. ​ Plan your strategy for an autoimmune relapse Therefore, you want to have a strategy to exit a relapse as soon as possible. You do this by monitoring what works for you when you feel good: What foods keep you in remission? Which supplements help you stay in remission? Identify the day-to-day lifestyle habits that keep you in remission. What sort of exercise routine keeps you in remission? Do you have personal and social interactions that help keep you in remission? The goal is to understand the key factors that may have caused the relapse and create a viable plan to help you get back into remission. ​ The autoimmune relapse strategy to get back into remission This may mean following a stricter anti-inflammatory diet for a while, taking some time off work, avoiding stressors, taking a break from your volunteer activities, and so forth. ​ Give yourself rest, relaxation, a calm environment, and an anti-inflammatory diet and lifestyle as much as possible. You may also find increasing doses of anti-inflammatory supplements such as liposomal glutathione, resveratrol, and turmeric help. However, relapse is NOT the time to experiment with new supplements. Nor is it the time to binge on inflammatory comfort foods. ​ You may need to lessen the intensity of your workouts if you exercise regularly. Some physical movement can help dampen inflammation in your body, but overexercising is inflammatory. Incorporate an appropriate amount of movement without overdoing it. ​ ​ ​ ​ ​ ​ ​ ​ ​ Common misconceptions about autoimmune relapses and remission Both conventional and alternative health care models largely operate under misconceptions and lack of education when it comes to autoimmunity. ​ As a patient, it is very important to understand the basics of autoimmunity and have realistic expectations. This will help you find the best healthcare professionals who understand your condition. ​ Different people have different reactions to autoimmune diseases and their treatments, even if they have the same disease. A personalized treatment plan developed through careful trial and error and close observation will help you stay in remission longer, minimize relapses, and get out of relapses faster. ​ Healthy expectations with autoimmunity Patient perspective: The goal is to get into remission and maintain it as long as possible without major flare-ups.     Dr. Hugh's Comprehensive Guide to All Things Gut Health Your gut’s influence over your health cannot be overstated. The trillions of microbes and colonies located in your microbiome are the manufacturers and managers of how you look, feel, and think. It makes up 70-80% (1) of your immune system, produces (2) a large amount of your “happy” neurotransmitter serotonin, and contains 10 times more bacterial cells than you have human cells! ​ You are, in truth, more bacteria than human. And researchers are quickly learning just how much it regulates all aspects of your health. ​ When your microbiome is healthy your health is likely to be quite good. However, many lifestyle behaviors, toxic exposures, and dietary choices can compromise gut health and lead to many downstream health issues. For example, when your microbiome is weakened or damaged, it can “switch on” a number of potential disease processes throughout the body that may, on the surface of things, seem to have very little to do with your actual gastrointestinal system. ​ That is why it’s vital to educate yourself on the microbiome. By doing so you are taking back control of your health. Read on to learn more about what happens when your gut is unhealthy, what causes poor gut health, natural ways to heal your gut, and more. This is your definitive guide to all things gut health. ​ Health Problems That are Triggered by Poor Gut Health Because your gut controls so many areas of your health, you don’t have to have typical gut symptoms to have gut problems. These are a few of the far-reaching health problems your gut has influence over. ​ 1. Autoimmune conditions As of now, there are around 100 recognized autoimmune conditions and about 40 other diseases that have an autoimmune component. Since your immune system mainly resides in your gut, it is no surprise that a damaged microbiome and leaky gut syndrome are preconditions for autoimmunity. ​ 2. Mental health disorders The gut-brain axis links your gut and brain, which is why, in the medical literature, your gut is actually referred to as “the second brain.” An unhealthy microbiome has been linked to mental health conditions such as anxiety and depression. (3) ​ 3. Poor immune health The opposite of autoimmunity (an overactive immune system), suppressed immunity can be gut-related, too. If you find yourself sick often, you’ll want to support and improve your microbiome health. Chronically low immune system health can be largely due to imbalances in the microbiome, especially due to an overgrowth of opportunistic bacteria, yeast, or fungus, or a parasite. (4) ​ 4. Heart disease Scientists recently discovered a possible link between the microbiome and cardiovascular disease. (5) Certain bacteria in the gut produce TMAO (trimethylamine-N-oxide), which is linked to a higher risk of heart attack and stroke. It is still unclear which microorganism produces these excessive levels of TMAO, but researchers are hoping that, in the future, manipulation of the microbiome species can help in the prevention and treatment of heart disease. ​ 5. Type II diabetes This chronic degenerative disease has recently been linked to microbiome disturbances. One study found that transplanting the microbiome of diabetic mice into healthy mice made the healthy mice diabetic as well! (6) ​ 6. Skin conditions Skin problems like acne , (7) psoriasis , (8) and eczema (9) all have microbiome and inflammatory-autoimmune components to them. For many, the missing link to healing their skin issues is healing their microbiome. ​ 7. Weight gain and obesity Weight loss resistance and obesity have been linked to an imbalance of bacteria in the microbiome. Studies in mice found that overweight mice had a higher number of the bacteria Firmicutes, while thin mice had a higher proportion of the Bacteroidetes bacteria. (10) In human cases, the beneficial bacteria called Lactobacillus rhamnosus was found to support weight loss in women. The microbiome factor may turn out to be a key component for many people seeking to lose weight their body has been holding on to for years. ​ 8. Acid reflux and GERD Millions of people suffer from acid reflux, or the more serious GERD, and these problems have been correlated with a microbiome dysfunction called SIBO, or small intestinal bacterial overgrowth. ​ 9. Cancer A fascinating study out of the University of North Carolina suggests that damage and inflammation of the gut severely decreased the variety of bacterial species in the microbiome. (11) This loss of microbiome diversity allowed a pathogenic bacterial overgrowth of E. coli. Eighty percent of mice with E. coli infection then developed colorectal cancer. This suggests that there may be a cancer-microbiome connection, and I expect researchers may find more evidence of this connection. ​ 10. Asthma and chronic sinus infections Dysbiosis (imbalance) of microbiome bacteria and an overgrowth of Corynebacterium tuberculostearicum, were shown to be a frequent underlying culprit for asthma and chronic rhinosinusitis (CRS). (12) ​ 11. Constipation and/or diarrhea This is obvious, and is digestive so it doesn’t exactly go on this list, but I still feel like it’s important to mention that one study found that there was significantly lower amounts of the bacteria Prevotella and increased levels of Firmicutes in constipated patients. (13) Interestingly, the conventional probiotics that people take, Lactobacillus and Bifidobacteria, were not decreased in the microbiomes of the constipated patients, suggesting that supplementing with these may not be the solution to that issue. ​ What causes poor gut health? Everyone’s microbiome configuration is unique, kind of like a fingerprint, but there are certain elements that can cause trouble for most of us. While every person is different, these are the top lifestyle and medical contributors to an unhealthy gut. ​ 1. Poor diet Like it or not, food can either fuel health or fuel disease. One very common way food fuels disease is by damaging the balance of the microbiome. Processed and sugary foods are the most obvious culprits because they feed the more pathogenic types of bacteria as well as Candida albicans, but underlying food sensitivities to even so-called healthy foods can also lead to gut-damaging inflammation. Grains are one common example – even gluten-free grains and whole grains contain amylose sugars that “bad” bacteria and fungi love to consume, and that contribute to inflammation. The best diet isn’t always obvious, and depends on your personal microbiome makeup, genetics, and lifestyle. ​ 2. Gut-meddling medications Medications may be necessary (although many people take them unnecessarily), but even the ones that help you can have side effects—have you educated yourself on what they are? One of the most common side effects of both prescription and over-the-counter (OTC) medications is compromised intestinal barrier. In other words, medications can make your gut more permeable, leading to “leaky gut syndrome.” A few of the most notorious culprits: Antibiotics can save lives, but frequent use and overuse of these drugs kill gut bacteria without distinguishing between good and bad. With more good guys gone, pathogenic bacteria and fungi can take over, especially if you are not making efforts to restore the balance through probiotic supplements or fermented foods. Non-steroidal anti-inflammatory medications like ibuprofen, naproxen, and aspirin, relieve pain by blocking the enzyme cyclo-oxygenase, but this also inhibits it from doing its important job of protecting your stomach from the corrosive effects of its acid. The result can be an increase in intestinal inflammation and consequent permeability. Research estimates that 65 percent of people who consistently use NSAIDs have intestinal inflammation and 30 percent have ulcers. If left unchecked, gut permeability can trigger an autoimmune response. ​ 3. Chronic stress We are all meant to handle stress now and then, but when stress becomes chronic and you experience consistently high levels of the primary stress hormone cortisol along with decreased oxygen delivery to the gut, damage can be the unfortunate result. Blame the gut-brain axis because the emotional turmoil of chronic stress can directly impact gut inflammation. ​ 4. Alcohol overuse A glass of wine every once in a while probably won’t do much to your gut in the grand scheme of things (unless you are already experiencing inflammation and severe gut dysfunction). However, consistent alcohol consumption in even the healthiest people can be an intestinal irritant, as well as suppressing the hormones that protect against inflammation and gut permeability. ​ 5. Gluten and Grains The negative impact of gluten is well documented now, but in a few years, I believe research will confirm the similar, possibly even worse, negative impact of all grains, including those that are gluten-free. With their abundance of amylose sugars that cause inflammation, anti-nutrients such as lectins and phytates that bind to the intestines (14) and make nutrients inactive (15) in the body, and a low nutrient density trade-off for the calories that contain (especially when refined), grains can cause a wide array of damage to your gut and your general health. ​ 6. Autoimmune conditions Chronic inflammation can destroy the integrity of the gut lining and cause undigested food particles and the toxic by-products of digestion to leak out of the gut into areas where they are not meant to be. This can cause your body to mount an attack against these perceived “invaders,” increasing levels of pro-inflammatory cytokines that destroy your gut lining to even higher levels, in a vicious cycle of inflammation. ​ 7. Hormone imbalances Imbalances in the hormones estrogen, progesterone, testosterone, thyroid hormones, and cortisol have all been linked to sluggish healing of intestinal damage. This can lead to leaky gut syndrome, as the chronic inflammation in the gut lining damages it, causing it to become more permeable. This may explain why you may not be able to heal despite your best efforts—it’s those pesky hormones! Many people need to focus on balancing their hormones before they can successfully heal their guts. ​ 8. Imbalanced blood sugar When blood sugar skyrockets due to excessive carbohydrate intake or insulin resistance, compounds known as advanced glycation end products, or AGEs, go sky-high too, which can increase the permeability of your gut as well as inflict free radical damage on other tissues, accelerating aging. ​ 9. Neurological problems Your gut and brain are forever linked. They were formed from the same fetal tissue while you were growing in your mother’s womb. They continue this special bond throughout your life through a connection known as the gut-brain axis . Because of this bond, brain problems like depression, anxiety, autism, ADHD, stress, and dementia can lead to leaky gut syndrome and vice versa. ​ Types of gut problems Gut problems don’t always look the same. Even if your symptoms look similar to another person, healing can only truly begin when you uncover what is happening underneath the surface. ​ 1. Leaky gut syndrome An intestinal lining damaged by years of unhealthy eating, food intolerances, stress, and toxins, can allow undigested food particles and bacterial endotoxins called lipopolysaccharides (LPS) to pass out of the digestive tract and into the rest of the body where they don’t belong. Sensing an “invader,” the body can react with systemic inflammation that can become chronic. In functional medicine, leaky gut syndrome is considered a precondition for autoimmune diseases and many other health problems and requires intervention to heal the gut lining in order to stop the inappropriate immune reaction. ​ 2. Bacterial imbalances Your microbiome contains a delicate balance of bacterial and fungal species that live in symbiosis with you, but some of those species are pathogenic and can cause health problems if they are allowed to overgrow, crowding out more beneficial species – a condition called gut dysbiosis. ​ This can lead to health-damaging conditions like small intestinal bacterial overgrowth (SIBO ) which in turn have been linked to a wide variety of health problems, from irritable bowel syndrome to autoimmune disease. For example, anxiety and depression have been linked to lower levels of Lactobacillus helveticus and Bifidobacterium longum, and weight gain was linked to higher amounts of Firmicutes. ​ Ironically, when someone with dysbiosis or SIBO starts to eat healthier, by increasing vegetable intake for example, it can cause an increase in gut symptoms like constipation and bloating. But it is the underlying microbiome issue, not the vegetables that is the root problem. ​ 3. Histamine intolerance Another issue I often discover in patients with leaky gut syndrome and bacterial overgrowths is histamine intolerance, which is a dysfunction or deficiency of the enzymes that break down histamines – the chemicals produced during an allergic reaction. In people with histamine intolerance, foods that naturally contain histamine or trigger the release of histamine in the body, become problematic. Without the enzymes to effectively get rid of excess histamine, the overflow can cause digestive problems like heartburn, acid reflux and stomach pain after eating, among other issues like anxiety, headaches, hives, and asthma. ​ 4. Yeast overgrowth We all have some yeast in our gut microbiome, but overgrowths of yeast such as Candida albicans can cause chronic low-grade inflammation and immune stress. People with an already weakened immune system or an autoimmune condition can find candida overgrowth to be a trigger for more health problems. ​ How do I know if my gut is healthy? While you may suspect you have gut problems because of your symptoms, without running any labs, you won’t know for sure. These are the labs that I typically recommend to my patients, to determine if any of these gut problems are a factor in a particular health case: ​ 1. Gut permeability labs I always run tests for Zonulin and occluding antibodies. These two proteins control gut permeability, and the presence of antibodies can indicate damage to the tight junctions that keep your gut lining sealed. I also test for actomyosin antibodies, which can indicate destruction of the gut lining, and lipopolysaccharides (LPS) antibodies, which can also indicate leaky gut syndrome, if these antibodies are not contained within the digestive tract. ​ 2. A comprehensive stool analysis This test can uncover everything from the presence of parasites to bacterial imbalances to conditions like candida overgrowth or SIBO (small intestinal bacterial overgrowth) or SIFO (small intestinal fungal overgrowth)—any of which can negatively impact gut health. ​ ​ How long does it take to heal your gut? Let’s begin by imagining the surface area of your gastrointestinal tract. If you spread it all out, it would cover about the size of a tennis court! This internal “court” is covered in special cells called enterocytes that constantly regenerate so that you get an entirely new gut lining every two to three weeks. (16) ​ If you are healthy and don’t have any chronic conditions like autoimmunity or inflammation, and if you don’t have any food sensitivities, you could heal a not-so-perfect gut in as little as two weeks or as long as 12 weeks, studies suggest. (17) In fact, a study from Harvard, published in the medical journal Nature, found significant changes in gut bacteria actively happening just three days after a dietary change! Unfortunately, most people who are trying to heal their guts do have other health issues that make healing happen more slowly. If you have chronic inflammation, Lyme disease, viral infections, blood sugar issues, adrenal fatigue, SIBO, an autoimmune condition, histamine intolerance, candida overgrowth, or leaky gut syndrome, it’s going to take longer to get you back on the right track. You are on what I call the autoimmune-inflammation spectrum, so you are going to have to come at your healing from multiple directions at once to get results. ​ What is the autoimmune-inflammation spectrum? Autoimmune disease doesn’t just pop up one day out of nowhere. It begins with the slow burn of inflammation, which can simmer and spread for years before you get a diagnosis. This means if you have some inflammation, you don’t necessarily have a diagnosable autoimmune disease, but you could be headed in that direction. Did you know that to be diagnosed with an autoimmune disease, your immune system has to have already destroyed a significant amount of tissue (such as the brain, gut, or thyroid)? For example, there has to be a 90 percent destruction of the adrenal glands to be diagnosed with Addison’s disease (a disorder in which the adrenal glands don’t produce enough hormones) and massive destruction of small intestine tissue to be diagnosed with celiac disease (CD). (18) ​ This is why I look at this process as just that, a process, not a state. This process has, three stages: 1. Silent autoimmunity: You have positive antibody labs but you have no noticeable symptoms. 2. Autoimmune reactivity: You have positive antibody labs and symptoms. 3. Autoimmune disease: You have enough body destruction to be diagnosed with a specific condition. ​ A large percentage of my patients fall under #2. They have labs that are a little off, and they have uncomfortable symptoms, but they don’t have a diagnosis and have been told by their conventionally trained doctors that they are “fine.” Inflammation and symptoms are not “fine,” in my opinion. In these cases there is almost always some gut dysfunction and these patients are quite likely headed towards much more serious health issues if they do not intervene in the process. ​ For example, researchers are now finding what we’ve been saying for decades in functional medicine: food reactivities like gluten sensitivity are just the beginning stage of one end of the larger inflammation spectrum, with autoimmune conditions like celiac disease (CD) on the opposite end. ​ Only around 10 percent of people with CD have obvious digestive symptoms. Instead, they end up struggling with other seemingly unrelated symptoms like skin problems, anxiety, or depression. Therefore only about 5 percent of celiacs ever get diagnosed which leaves close to 3 million Americans with celiac disease who have no idea that they have it and another 15-20 percent with gluten sensitivity. (19) It can take up to six months just to begin reducing inflammation and antibody levels that can result from eating a reactive food like gluten. (20) ​ For anyone struggling on this severe end of gut dysfunction with chronic inflammation, leaky gut syndrome, or an autoimmune condition, healing can take anywhere between 12 and 24 months before true, sustainable healing is achieved. While many people see improvements each month, long-term healing can only be accomplished after this one- to two-year period of implementing various natural healing tools. ​ How to heal your gut naturally 1. An elimination diet An elimination diet is my gold standard for uncovering hidden food intolerances. In order to heal your gut, you need to stop eating foods that continue to damage your gut and increase inflammation, but you can’t know what foods are irritating to your individual gut until you remove all potential irritants for a certain amount of time and slowly reintroduce them one at a time, monitoring your body’s reaction. This will allow you to determine which foods your body loves and which foods your body hates. Rotating your food Variety is the spice of life as well as the salve to an inflamed gut. Eating many types of foods will not only give you a wide variety of much-needed nutrients, and rotating these foods to avoid having any one food too often will help you heal and keep your immune system balanced. A good rule of thumb is to never have any one food more than once a day, or even better, no more often than every three days. For example, if you love leafy greens, have Romaine lettuce one day, kale the next day, and collard greens the day after that, before rotating back to Romaine lettuce. ​ 3. Taking probiotics Probiotic-rich fermented foods like sauerkraut, kefir, and kimchi will reinoculate your microbiome with good bacteria—especially important if you’ve recently had a round of antibiotics or been under a lot of stress. A probiotic supplement will also give your gut a much-needed boost of essential bacteria. ​ 4. Drinking bone broth regularly This superfood brims with collagen and minerals that can soothe and repair a damaged gut. Sip it alone as a warm drink or use it as the base for soups and in other recipes . I recommend making bone broth yourself at home in a slow cooker or pressure cooker (it’s easy), using bones from grass-fed cattle or organic chickens. If you don’t want to make it yourself, Bare Bones and Kettle & Fire are both great options. ​ 5. Eating foods with L-Glutamine L-glutamine is considered a conditionally essential amino acid because your body uses so much during times of intense physical stress. Essential amino acids must be obtained from diet whereas nonessential amino acids—an example being D-glutamine—are synthesized by your cells. L-glutamine is essential for maintaining the health and growth of enterocytes in your gut since it is the preferred fuel source of these cells. So if you want to win the battle against leaky gut syndrome, L-glutamine should be a go-to resource in your toolbox. ​ There are many studies that support L-glutamine’s ability to improve gut permeability. (21) One study, in particular, looked at a group of 107 children and how they responded to L-glutamine. Half of the children took L-glutamine and the other half took a placebo. In just 120 days the children who received the L-glutamine supplement had improved intestinal barrier function. (22) ​ It’s also evidenced that increased glutamine levels can protect against gut permeability. In a study of 20 patients with intestinal permeability and in need of parenteral nutrition (total intravenous feeding), those whose food was enriched with l-glutamine saw no deterioration in gut permeability or mucosal structure whereas the other group did. (23) ​ Research has even shown that L-glutamine can reduce inflammatory bowel disease symptoms including in people with ulcerative colitis! (24) After four weeks, patients receiving an L-glutamine supplement saw a reduction in symptoms as well as an increase in Bifidobacterium. Surprisingly enough, once patients stopped the use of glutamine their symptoms returned. That just goes to show the powerful connection between L-glutamine and your gut. ​ There are many food medicines that contain L-glutamine such as grass-fed beef, bone broth, and grass-fed dairy. For those who want a plant-based option, red cabbage is an abundant source. Eating cabbage in the form of sauerkraut will increase its gut-healing abilities since the fermentation provides your gut with probiotics that also help make L-glutamine more bioavailable. ​ If you decide to start supplementing, anywhere between 2 and 5 grams per day is typically a good dose for most people. You may also want to look for an l-glutamine powder rather than capsules since it can be easier for your gut to digest. ​ 6. Trying intermittent fasting By going extended periods of time without eating, you’re able to give your digestion a much-needed break. There are many ways to do this, such as fasting every night for at least 12 hours (many people do 16 hours) or fasting for one day every week. ​ 7. Managing your stress If you are feeding yourself a giant slice of stress every day, all the healthy food in the world isn’t going to help heal your gut. Chronic stress can suppress the immune system, decrease blood and oxygen flow to the intestines, and contribute to gut lining permeability. Making time to de-stress through pressure-relieving activities like tai chi, yoga, or meditation can make a huge difference in your stress levels and in turn, your gut health. ​ 8. Cooking with coconut oil How did we ever live without coconut oil? This versatile superfood is full of the healthful saturated fats that are integral to gut healing, as well as lauric, capric, and caprylic acids that have important antimicrobial, anti-fungal, and antiviral properties. This real food medicine gently cleanses your GI system but is also great externally to nourish your skin. Choose organic, extra virgin, cold pressed varieties for maximum benefit. ​ 9. Eating more cooked foods Raw foods are packed full of nutrients but they can also be difficult to digest. Cooked foods decrease the amount of work your digestive system needs to do to break down food because some of that has been done for you through the cooking process. Even easier to digest are pureed foods like smoothies and pureed soup. Going easy on your digestion can help reduce bloat and lead to a healthier gut. As you heal, you will be able to tolerate raw foods better. ​ 10. Taking targeted natural supplements Nobody wants to take a giant fistful of vitamins every day, but there are a handful of gut-healing supplements I often call upon to help my patients in digestive distress. These can help speed healing and make a big difference in symptoms: ​ Colostrum : The lactoferrin in colostrum works as a prebiotic to feed good bacteria and fuel its growth. It also promotes cell growth in the intestines to repair a damaged gut. Slippery elm: This natural botanical works as a demulcent to reduce inflammation in the gut. You can find this in tea or supplement form. Turkey tail: This adaptogenic mushroom works wonders against gut overgrowths like SIBO and candida overgrowth. Try it in a warm drink. Deglycyrrhizinated licorice: Sip on licorice tea to soothe and heal your gut lining and ease digestive trouble. Marshmallow root: This root supports the repair of a damaged gut lining by coating the stomach to protect it against increased inflammation. You can find this in tea or supplement form as well. ​ 11. Drinking Lemon Water Lemons contain ample amounts of phytonutrients, vitamin C, and fiber which are all needed for supporting gut health. Vitamin C is a powerful antioxidant that helps to lower inflammation in the gut and boost the immune system. It also works as a natural antimicrobial to bring balance to the bacteria in the microbiome. ​ Lemons are also high in a type of fiber called pectin. This is good for your gut bugs and helps to promote healthy gut bacteria balance and encourage growth of beneficial bacteria. Multiple studies have shown that these particular fibers, like the ones in lemons, stimulate the growth of important probiotics in the microbiome like bifidobacterium. (25) ​ The whole-food fusion of vitamin C, prebiotic fiber, and phytonutrients also lends itself to leave the drinker with a cleaning-like effect, especially if drunk first thing in the morning on an empty stomach. ​ In order to really get the most positive impact on your gut health, I suggest not using lemon juice as a stand-alone tonic. Try to keep as much of the lemon pulp as you can with the lemon juice in your water since the pectin fiber and phytonutrients are mainly found in the lemon pulp. Also, make sure the water is either warm or cold, not hot, as the hotter water denatures the Vitamin C. ​ 12. Taking Apple Cider Vinegar Research has shown that vinegar can mildly lower the growth of gram-negative bacteria like Pseudomonas aeruginosa and Staphylococcus aureus. (26) These bacterial colonies are higher in bacterial endotoxins called lipopolysaccharides (LPS). Higher LPS levels are implicated with a whole slew of inflammatory health problems as well as leaky gut syndrome. ​ ACV has also been shown to have anti-yeast, anti-fungal, and antiviral benefits which are all helpful at supporting the microbiome and immune balance. (27) ​ Another potential benefit of apple cider vinegar is its ability to prove indigestion and heartburn. One of the most common causes of heartburn I find in my patients is low stomach acid or hypochlorhydria. I have found that many patients see improvement with their heartburn, acid reflux, and overall digestion when they take a small amount of apple cider vinegar with their meals. ​ Overall, I usually always see the best results when using ACV in it’s raw, unfiltered form with the “mother” included. You’ll be able to see the mother as a sediment in the bottle but it usually always also says so on the label. Since ACV is very acidic, you may find that diluting it with water or juice can make it easier to swallow. Drinking straight apple cider vinegar can damage tooth enamel and the throat. Typically, I find people to well with 1 to 2 tablespoons mixed with 1 ounce of water. ​ 13. Taking Digestive enzymes Digestive enzymes are proteins that break down the food you eat into smaller pieces that are easier for your body to absorb, utilize, and turn into energy. Some people have food intolerances (like lactose) where they lack the enzymes to break down this specific protein or don’t make enough digestive enzymes which can slow digestion and lead to uncomfortable symptoms like bloating, cramping, and gas. This is not uncommon, especially in people who are over 50, have low stomach acid, or you have IBS or IBD. ​ While digestive enzymes aren’t the go-to solution for those with specific intolerances, they can be helpful to have on hand for when you come in contact with a food you weren’t expecting to eat or if you need a little help with enzyme production. ​ 14. Eating more fiber Fiber is essential for a healthy gut. Not only does it help you build up good gut bacteria, it keeps you regular and having healthy, solid bowel movements. Fiber can be found in vegetables like artichokes, carrots, and broccoli. ​ 15. Exercising Exercise is great for all areas of your health, but it can also increase beneficial bacteria in your gut and overall bacterial diversity. (28) ​ 16. Eating foods with digestive bitters Digestive bitters are herbs that have a bitter taste including burdock root, bitter melon, and dandelion. Digestive bitters are taken with meals and work to stimulate the production of digestive enzymes that help break down food and make the digestion process easier on your gut. You can find digestive bitters in herbal tincture form but I personally love adding bitter greens into soups, salads, and stir-frys. ​ 17. Eating ginger Ginger has been used for thousands of years to soothe stomach problems due to its anti-inflammatory properties. It also helps to alleviate heartburn and acid reflux as well as stimulate the production of stomach acid. Try adding ginger to more recipes or cut up fresh ginger root to make a gut-soothing tea to sip on. Nearly all quinolone antibiotics in use are fluoroquinolones, which contain a fluorine atom in their chemical structure and are effective against both Gram-negative and Gram-positive bacteria. ​ Fluoroquinolones “kill bacteria by blocking enzymes which normally untangle DNA during cell replication. Usually, these enzymes cut DNA’s double helix, pass another part of the strand through the gap, and then mend the cut. ​ But quinolones bind to the enzymes, preventing them from mending their cuts. In the 1980s, researchers added fluorine atoms to the quinolones’ structures. This allowed the antibiotics to penetrate tissues throughout the body, including the central nervous system, and boosted their effectiveness against a broad range of bacterial infections.” (2) ​ Fluoroquinolones are of value in certain infections, including some life-threatening ones, where alternative antibiotics are not sufficiently effective. ​ But fluoroquinolones, as with many antibiotics, have been overused.
